HealthEmove wants the best possible care for people often changing healthcare providers


Our mission is to give health care workers access to the medical record of a 'person on the move' visiting their clinic for care. With insight into the medical history of their patients, healthcare workers can make the best decisions.

Patients can decide who has access to their record. Both the patient and the healthcare provider can add data to the medical record, but you can see who added the data. 

We create HealthEmove in co-creation with vulnerable groups in Amsterdam and their healthcare providers. We intend to designate certain organizations and their health care workers to edit the information in the person's file so that the information is up to date and gives a quick and useful overview for other health care workers who have not had the chance to work with the app.

How does it work?

1. A patient will give permission to show her medical file to the healthcare worker via their HealthEmove account. Please provide your name and email address to the patient.

2. You will receive an email to access the health record of the patient

​

No time to fill out your e-mail address? Ask a patient if you are allowed see their record on the phone of the patient. Specifically navigate to the 'files' section as all pictures are stored there.

3. Create an account on HealthEmove. 

​

4. The healthcare worker can access the medical records of the person in front them, including: diagnoses, medication and allergies. You can even send a message to the patient.

 

5. At the end of the consultation the health care worker can

(1) either put information directly into the HealthEmove record

(2) allow a person on the move to take a picture of your own medical record system

(3) send a pdf/prints screen of your own medical record to the patient.

As healthcare providers we must work together to  provide the best possible care for people on the move and eventually safe time. 

To avoid double documentation, we ask patients to take photos of referral letters, prescriptions and pictures of the screen of healthcare providers.

You will find information about

Medication, vaccinations, diagnosis, and information about the patient. Specifically look at the FILE section. All pictures are stored at the file section.

If the patient wants to share specific information with you, click on care plan "about me".
